We operate http://www.SPEDform.com (the “Site”). This page informs you of our policies regarding the collection, use and disclosure of Personal Information we receive from users of the Site.
We use your Personal Information only for providing you the ability to create forms concerning education. By using the Site, you agree to the collection and use of information in accordance with this policy.
Information Collection And Use
While using our Site, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you and/or your child. Personally identifiable information (“Personal Information”) may include, but is not limited to your name, address, email, phone number, names and similar information for other family members, educational information concerning your students, and the like. Registered users have the ability to save and to delete any Personal Information to or from our server.
A primary goal of this Site is to enable you to properly interact with school districts and other third parties. Some forms and pages in the site explicitly request contacts in the school district and will, upon your choice, email the form to the school district with your information. Other forms are designed to interact with third party agencies (e.g., Office of Civil Rights, State Complaints, etc.) and with full disclosure on the form, will submit the form and your information to said third party. Other pages of the Site, such as surveys, specifically inform you how the information will be used (as does this policy statement, below).
We will not respond with any personally identifiable data to a request from any third party unless there is a court order mandating that we do so.
Like many site operators, we collect information that your browser sends whenever you visit our Site (“Log Data”).
This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages and other statistics.
Cookies are files with small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a web site and stored on your computer’s hard drive.
Like many sites, we use “cookies” to collect information. You can instruct your browser to refuse all cookies or to indicate when a cookie is being sent. However, if you do not accept cookies, you may not be able to use some portions of our Site.
Surveys & Contests
From time-to-time our site requests information via surveys or contests. Participation in these surveys or contests is completely voluntary and you may choose whether or not to participate and therefore disclose this information. Information requested may include contact information (such as name, school, email address), and demographic information (such as zip code, age level). Survey information will be aggregated if shared publicly or with a third party and all identifying information will be removed. Contact information will not be shared and only used by this Site to contact you as defined in the survey or contest.
The security of your Personal Information is important to us, but remember that no method of transmission over the Internet, or method of electronic storage, is 100% secure. While we strive to use commercially acceptable means to protect your Personal Information, we cannot guarantee its absolute security.
